• I acknowledge that I have read and understand the information contained in the adoption packet or agree to review and familiarize myself with the materials provided.• I authorize BCHS to use my adoption photograph for social media and promotional purposes.• If my adopted cat or dog requires spay/neuter surgery, I acknowledge that BCHS will not contact me regarding the procedure unless otherwise necessary. I agree to pick up my animal between 4:00 p.m. and 5:00 p.m. on the designated pick-up date.• If my adopted cat receives an FeLV/FIV test in conjunction with surgery, I acknowledge that BCHS will contact me only if the test result is positive.
